Perhaps the least fair fight to date. There are few monsters as iconic to D&D as the Owlbear.
My daughters had their friends over to play D&D. They managed to befriend an Owlbear and I've never seen a group of kids have so much fun playing D&D.
When I was their age we would not have hesitated to slay it and take its stuff. Instead the girls, of course, had to make it their pet.

I think my Fiend Folio just fell open to penanggalan the first time I sat down with it. The description and illustration bored into my head and penanggalans occupied my teenage nightmares for quite a while. Flying heads rules our D&D worlds for months.

Purple worms are the best A/D&D monsters of all time. Fact. They're iconic. It's unfair to pit another "P" monster against them for the same reason "Owlbear" was the obvious "O" choice. Some monsters are just iconic to this brand.

I voted Quickling because conceptually I like them the best - the monster catalogue could use a better variety of evil fae - but looking back at their actual stats and powers I definitely wouldn't use them as-is. I always found Quasits redundant, personally. I never saw any reason why an Imp needed to be Hell-specific and couldn't just be a generic Lower Planes vermin/servitor creature. I feel the same way about Lemures vs. Manes.

Dragon Magazine #86, one of the best issues in the golden age of Dragon Magazine, has a great article on special familiars. But none of them were as cool as the Quasit. So, it's no surprise they won (not that the challengers were particularly strong).

Roper's iconic, but I love the idea of the Revenant. Imagine that NPC your murder-hobos killed for no good reason returning with an eldritch strength powered by vengeance a year later. He's basically the Crow, and the Crow is badass.

The rat in the Monster Manual II is not the iconic Giant Rat, but rather is two entries, one for the rarely-used Ordinary Rat and the other for the even-more-rarely-used Vapor Rat.
I had actually forgotten the latter even existed, probably because there is no picture of them. The entry is way too long for what it is, but includes some interesting details - they can dwell in the cloud islands of cloud giants, can assume Gaseous Form at will, and turn into a small Stinking Cloud spell when killed.

Normal rats can also eventually become a small stinking cloud, especially if you forget about the glue trap behind the fridge for about three weeks.

Stirges are an iconic D&D monster. They were included in the radio drama shared on Peterson's blog. It's a tough call between them and Sahuagin for me but I'd include Stirge.
