Since combat is abstracted in OD&D how do you handle attacks with the alternate combat system? separate die rolls for each attack?

That's what I do. I my games it generally only comes up if Fighters of higher than 1st level are taking on 1 HD creatures. In that case the player usually just rolls multiple d20s at once so it doesn't slow things down much.

For the bulk of monsters (and humans,) I only allow an attacker to focus on one opponent at a time, so I prefer to represent "multiple attacks" as a bonus to the attack roll. So, character wielding two daggers gets a +1 to attack (and no shield benefit.)
Creatures with multiple heads or otherwise capable of focusing on multiple opponents get multiple attack rolls. Hydras are dangerous!
I consider the fighter's special ability to mainly be the result of sequential attacks when facing insignificant opponents, so that could be an exception to the "one attack roll" rule... but I've been toying with eliminating multiple attack rolls even there. Instead, when facing nearly identical ordinary opponents of 1 HD or less, a successful attack roll means the fighter injures a number of opponents equal to the fighter's level. Either roll that many dice of damage and apply one die to each opponent, or just roll one die and do that much damage to each opponent.

For fighting with two weapons I have the same rule just a +1 to attack and I was trying to figure out something similar. A one roll the multiple damage dice spread out might be the way to go, but it wouldn't work to well with one opponent against a Hydra. So monsters such as trolls with a claw/claw/bite would get one roll but then roll 3d6 for damage

I assume that multiple attacks gives the character multiple dice rolls. Gives them something to do so they don't get bored.
