Cool. It'd be helpful to have some different pages, blank pages, lined pages, maybe some fields for important NPC's met, other relevant details.
I don't think it needs the D&D title and the old TSR logo, you can own it with your own title and logo - just keep the layout and style.

A number of years back, Troll Lord released a "player chronicle" for Castles & Crusades that was pretty slick. Sadly, I think it was one of those limited print stretch-goal only things and I don't think they ever released it widely in PDF. It was pretty much just what you guys are looking for, and would easily work for OD&D. I'll have to see if I can track it down and/or get Steve to offer it up as a PDF.

I spoke to Steve this morning about getting the Player Chronicles released via PDF. I think they'd be perfect for an OD&D character. The first two pages are a basic character sheet: Name, Class, Race, Alignment, hit points by level, ability scores and mods, languages and notes.
Thereafter, it's divided into identical 4-page sections. Each is intended to chronicle one adventure, and includes date, level, BtH (which wouldn't translate DIRECTLY to OD&D, but stands for "base to hit"), location or starting point, Hit Points, AC, Armor Type, Weapons, Consumables, Spells memorized, Equipment and Treasure, Significant Monsters, and Campaign Events.
There's room in a single book for 7 adventures (though a PDF might just have one, allowing you to print as many as you like).
The last 6 pages are for recording other PCs, important NPCs, and records of "The Noble Dead."

Hmmm. Just musing outloud. It might be useful as an in game journal, for something like this, but maybe containing resource pages to mark off torches, turns, spikes used etc.

My OD&D game is going well, so I took some time last weekend to work up a notebook to help track the events and growing cast of NPCs. I took a dotted journal I bought for $5 at Michael's a while back, set up different sections for House Rules, PCs, NPCs, Retainers, Calendar, and so on. Here's a page from the House Rules section... Here is the first pages of the current PCs. (There are more pages in the section in case some of these die!) And here is an idea I stole from a current Kickstarter called The Worldbuilder's Notebook. I set up little boxes along the edge of each page, and then color coded them according to the section so I could find them quickly... I used it Wednesday night and it worked gangbusters.

This is lovely. If we weren't playing online and had already set everything up on Google docs (character sheets on spreadsheets and such) I'd be printing these out to bring them to my players.
