Beware, some of these thoughts are not very well tested, but I thought I'd throw them out there anyway.
Firstly: the Man to Man table. I use this in a very specific way. I use it when both combatants have EXACTLY THE SAME MAN TOTAL. E.g. if you have a 3rd level Conjuror (2 Men) vs a Ghoul (2 HD) you would use the Man to Man table, and taking it in turns to take their hits until they run out of attacks before allowing the rest of the combat to flow on.
This allows for this sort of heroic combat, but only when both antagonists are of the same power. This makes more sense to me as the Man to Man tables take little account of individual skill, only weapon vs armour.
I don't believe it was the way it was meant to be used, I just prefer to interpret the phrase Man to Man in that way.
I always use Man to Man for missile fire though.
The Fantasy Supplement I use as it is when it is relevant.
This is the mostly untested alternate Mass Combat system (or for anything that does not fit into Man to Man or Fantasy as used above):
Both sides roll 1 dice for each HD or Fight Capability Men (however you want to refer to it). Total up each sides dice and compare. The higher total deals the difference in damage to its opponents hit points.
I see this as a better comparison of skill between opponents than the existing Chainmail one (though I still enjoy using Chainmail). Any attack bonuses are added to your attack total, and any defence bonuses are subtracted from your opponents attack total.
This is unashamedly Tunnels & Trolls based. The way in which armour and shields etc works in this is somewhat different though.
Both shields and off-hand weapons give you an extra d6.
Shields give you rerolls for every 1 that you roll. You may use each of them to reroll either any one of your dice or any one of your opponents dice. If you roll any new 1's these do not give you extra rerolls.
Off-hand weapons have the same effect, but you must remove the dice that gives you the reroll from your pool in order to get the reroll.
Then, if you are wearing armour you may remove dice from your opponents attack that show certain numbers or less:
Leather - 1
Chain - 2
Plate - 3
