There's a troll's den in Skyrim that has almost that exact setup, on the road to Iverstead. Would also work well as an actual dungeon entrance, too.

A. designed to collapse on you once you try to enter the dungeon.
B. actually a treant in disguise.

Many today are afraid. Others just shake their heads, continue with their work, and sharpen their blades. A few still blaze trails. Yet humanoids ravage the borders. Undead make cave of the ruins. Trolls prey farther out as do manticores, ettins, dire wolves, and wereboars. And, wyverns scry from high above the hillocks.
Hah! Now the dungeon entrance makes sense!

I hope you don't mind I found a couple of cool ones, the entrance to Bathory Cave: I was wondering about some of the Christian symbols present, then I read the article. It was used by Laszlo Bathory, not Elizabeth Bathory.

Those enclosed spaces remind me of the horror story of Nutty Putty Cave. Only google that if you have a strong constitution.

Much like a spelunker tragedy in Iowa when I was in college there 1987. A little bit of rain outside; a raging torrent in the cave resulted in two deaths, a student and the team leader.
