Great post here about playing Carcosa in a pre-Flood setting: From the Sorcerer's Skull: Antediluvian ApocalypticToday I found myself checking out a list of Aztec festivals and attendant party favors, as one does, and thought Pre-Columbian Central America might also make an interesting Carcosa-esque setting. Start off with the various New World polities arrayed against each other, with the prophesied arrival of a new group — the Teules — at some fixed point in the future, come to cast down the old civilizations and rule over them until the end of the world. But who are these newcomers ... ?

I've used the death gods from the Central American Mythos in my homebrew campaign, albeit with different names. One of them has become the chief deity in the great empire. The emperor himself (who may secretly be a snake-man) performs the daily human sacrifice.
There are some good ideas to incorporate into a Carcosa campaign.
