B. Portly made some very nice ones for his famed "Carcosa Boxed Set" and he might be persuaded to give or sell you some. Another fellow made a character sheet for Carcosa (original version) but I can't seem to locate it. It had a lot of tentacle-creepy goodness and was obviously a labor of love. I thought I had a copy on my hard drive but I can't locate it, and a goggle-search came up snake eyes.
