The first printing of the first Arduin Grimoire has an Erol Otus cover. The Forward page has the following: 'Special Note: the artwork for this supplement is the sole doing of one fine young artist: Errol Otus. I'm only glad I'll be able to say ten years from now, "I knew him when..."'
Otus's art and the above Special Note both disappear from subsequent printings. Does anyone know why?

Perhaps because Otus came to be so strongly associated with TSR's output?
btw: how is work going on the Carcosa update?
